Rehabilitation & Hospital Transition Support
Returning home after hospitalisation, rehabilitation, or another care setting can sometimes be challenging. Participants may need additional assistance while they regain their confidence, adjust to changes in their routine, or work towards becoming more independent.
Risewell Community Care provides practical support during this transition, helping participants settle back into their everyday lives in a safe and comfortable way. Our support can include assistance with daily routines, personal care, mobility-related activities, household tasks, community access, and other areas where additional help may be needed.
We can work alongside families, healthcare professionals, rehabilitation teams, and other relevant support providers to help maintain a consistent approach to care. Where appropriate, we follow professional recommendations and support the participant in working towards their individual recovery and independence goals.
Every person’s recovery is different. We take a patient and respectful approach, giving participants the time and encouragement they need as they adjust to life after hospital or rehabilitation. Our aim is to help make the transition home smoother while supporting confidence, independence, and day-to-day wellbeing.
